Hospice Outreach (previously Community Nursing Service)

The Clinical Nurse Specialists (CNS) provide specialist advice and support to the most complex patients, working closely with the Consultants in Palliative Medicine. Two Clinical Nurse Specialists are Independent Prescribers. The team supports patients and families through face to face assessment and review in outpatient settings, at home or virtually.

The CNS team provides specialist palliative care support, including holistic assessment, symptom management, psychosocial support and liaison with other health professionals involved in the patient’s care.

The team work closely with GPs, Community Nurses, Care Homes, specialist services such as oncology and RESPS teams and the Hospital Palliative Care Team.

Specialist Palliative Care Telephone Support Line

The Specialist Palliative Care Telephone Support Line is available 24 hours, 7 days a week to provide advice to patients, families and health and social care professionals regarding unmet palliative care needs for patients at home or in a care setting that is their home. This can include symptom management, psychosocial support and guidance and support at the end of life.

The service is supported by the interdisciplinary team Monday – Friday and out of hours by a Consultant in Palliative Medicine.
